Understanding Heat Rash (Miliaria)

Heat rash, or miliaria, develops when sweat ducts become blocked or inflamed. It can cause prickling, itching and small bumps during hot, humid conditions. Babies and people in occlusive clothing are particularly susceptible. Not every rash after sweating is heat rash, and fever or significant illness needs a separate assessment.

Common symptoms

  • Small itchy or prickly bumps in sweaty or covered areas.
  • Tiny clear superficial blisters in some forms.
  • Irritation around the neck, trunk or skin folds.

Causes and contributing factors

Heat, humidity, heavy clothing and prolonged sweating can obstruct sweat ducts. Occlusive ointments may worsen some cases. Infants have immature sweat ducts and can develop the condition when overheated.

How the condition is assessed

The heat exposure, clothing and rash pattern help guide diagnosis. A clinician considers other causes if lesions are painful, pustular, persistent or associated with illness.

Care and treatment considerations

Move to a cooler environment, reduce sweating and use loose breathable clothing. Avoid overheating babies or applying thick oily products to an active rash. Persistent irritation may need clinician-directed treatment rather than repeated application of strong steroid combinations.

When to seek medical help

Seek prompt care for fever with an unwell child, pus, spreading redness or symptoms that do not improve after cooling. Confusion, collapse or suspected heatstroke is an emergency, not a routine rash complaint.

Patient questions

Can heat rash spread between people?

No. Miliaria is related to sweat-duct blockage rather than a contagious infection.

Should I apply a heavy cream over every heat rash?

Not necessarily. Thick occlusive products can trap heat and sweat. Cooling and a suitable product choice matter more than applying multiple remedies.
