Understanding Epidermoid Cysts

An epidermoid cyst is a usually benign sac beneath the skin containing keratin. It often appears as a slowly growing rounded lump and may have a small central opening. It is commonly called a sebaceous cyst, although that name is not technically accurate. A new lump still needs assessment because other conditions can look similar.

Common symptoms

  • A rounded lump beneath the skin, sometimes with a visible central pore.
  • Slow growth and little discomfort when not inflamed.
  • Pain, redness or discharge if the cyst ruptures or becomes inflamed.

Causes and contributing factors

These cysts can arise from the upper part of a hair follicle or from trapped surface skin cells. Inflammation does not always mean bacterial infection. The diagnosis should not be assumed for every lump.

How the condition is assessed

A clinician examines the location, consistency and rate of change. An unusual, fixed, rapidly enlarging or recurrent lump may need additional investigation. Tell the clinician about previous drainage or attempted removal.

Care and treatment considerations

A quiet cyst may not need immediate treatment. If it is troublesome, complete removal of the cyst wall may be discussed after assessing inflammation and the site. Do not squeeze or puncture it: this can cause rupture, inflammation, infection or scarring.

When to seek medical help

Seek prompt assessment for a hot painful lump, spreading redness, fever or rapid enlargement. A hard fixed lump or an uncertain diagnosis should be checked rather than managed as a cyst at home.

Patient questions

Is every inflamed cyst infected?

No. Rupture can trigger inflammation without a bacterial infection. A clinician decides whether antibiotics or another approach is appropriate.

Will squeezing out the contents stop it returning?

Not reliably. The lining can remain and refill. Attempted squeezing also increases the risk of inflammation and scarring.
